Subir imágenes con php

Necesitaría que este código funcionara en mi sitio. Tengo todos los permisos de la carpeta pero aún así el último paso de “move_uploaded_file” me salta error.
Realmente no se qué hacer, las soluciones de internet llevan todo a lo mismo, sobre los permisos que ya tengo. Y revisando el administrador de archivos, no encuentro en ninguna parte la imagen a subida.
